FOOTBALL IS A PHYSICAL SPORT. I hereby give permission and approval for my child/ dependent, to participate with Malmat Soccer Excellence Academy. I assume all risks and hazards are incidental to such participation and hereby release the Academy and its employees from all liability arising from injury or injuries sustained by my child/ dependent, while participating in the related activities. I also understand that Malmat Soccer Excellence Academy assumes no responsibility for damage to or loss of property. I also authorize the Academy to make decisions regarding obtaining medical care for injuries and illness that may affect my child/dependent which might occur during their training session. I further direct all medical or hospital facilities to accept this document as authorization to render emergency care to my child/dependent, should it be deemed medically necessary.
